Required Materials

  4. Website URL(s) with production content live.
  5. Patient intake questionnaire (PDF export).
  6. Prescription disclosure copy (see template).
  7. Refund & shipping policy (see template).
  8. Privacy Policy & Terms of Service published on live domain.
  9. BAA with CloudHealth executed.

Sample Answers

"Describe your clinical workflow"

Patients complete an asynchronous intake questionnaire reviewed by a state-licensed provider within 24 business hours. Synchronous video is available on request and required for controlled substances. Approved prescriptions are transmitted to partner pharmacies via Surescripts.

"How do you verify patient identity?"

Government-issued photo ID uploaded at intake, matched against payment method. Selfie verification for controlled-substance workflows.
